Skilled Nursing is provided by an RN or LPN with training and experience in providing care in the home. The case manager or nurse communicates frequently with your physician and other health care members to update your plan of care. Our highly skilled nurses have extensive knowledge in evaluation of patient needs and treating and educating patients, family members and caregivers in the home setting. We offer a wide range of skilled nursing services in areas such as disease-specific management and education for conditions like heart failure, COPD, diabetes and orthopedic rehabilitation. We also assist in medication management and administration, and provide dietary and nutrition management. Services may also include performance of skilled nursing procedures such as wound care, lab draws and IV infusions.
Our services include Physical, Occupational and Speech Therapy.
Therapists will evaluate patient needs and recommend services that may enable patients to experience a higher quality of living. Services are provided by a licensed therapist or licensed therapy assistant under the direction of the therapist.
Certified home health aides are available to assist with bathing, personal care and activities of daily living.
Our medical social worker services may include short-term counseling services, referral to and coordination with community resources and assistance with living arrangements, finances and long-term planning.
